Automatically substantiate HSA/FSA transactions for purchases made through Truemed.

The Pay and Chase Problem

When members use their FSA cards for health and wellness purchases, TPAs face an operational challenge: these transactions require Letters of Medical Necessity (LMNs) for substantiation. Without them, TPAs must initiate “pay and chase”—manually following up with members to collect documentation.

This process is expensive and time-consuming:

  • Administrative cost: $5-25 per pay-and-chase instance in staff time
  • Member friction: Repeated emails and calls to collect receipts and LMNs

How Truemed Solves This

Truemed qualifies purchases at checkout, generating LMNs before the transaction even settles. When TPAs receive Truemed transactions in their settlement reports, all substantiation documentation is already available.

For each matched transaction, Truemed provides:

  • Patient name associated with the purchase
  • Approval date when the qualification was completed
  • Receipt URL for the purchase receipt
  • LMN URL for the Letter of Medical Necessity
  • LMN expiration date
  • Diagnosis categories with IRS-compliant category mappings
